Required Emergency and Health Forms

Prior to your child's first day of school, the Office of Child Care (OCC) requires the following forms be on file at our school. All enrollment forms for 2023-2024 should be submitted by August 15, 2023. These forms must be completed every school year and for each child enrolled. 

Please note, the OCC periodically updates the forms. Completed forms submitted prior to an update remain valid, but any old versions submitted after an update will not be accepted. Therefore, please be sure to check this website for the most recent form before filling out or forwarding to your doctor.  

Forms are valid for one year from the signature date and will need to be renewed if they expire prior to the end of the 2022-2023 school year (e.g., a Health Inventory dated July 2021, will expire in July 2022, and therefore an updated form would be required for attendance through the remainder of the school year). (NOTE: If your child is due for a wellness visit between August 1, 2023, and August 31, 2023, then please contact us.)

  • Emergency Form -  This form is updated annually. Complete all items on Side 1 and Side 2 as applicable. 
    • Parents should use their name if they are the only authorized to pick up their child on a daily basis. It cannot be left blank. 
    • When parents cannot be reached, a local person needs to be one of the contacts on the list. 
    • If your child has a condition(s) which might require emergency medical care, they doctor needs to put care instructions and sign/stamp Side 2. 
  • Health Inventory Part I, Part II, and Blood Lead Testing Certificate:
  1. Part I is to be completed and signed by a parent.
  2. Part II must be signed by your child’s doctor, regardless of whether the medical office opts to provide a computer printout of the required information. If your child's doctor attaches a printout, then it must include all of the information requested on the original form and must be signed, dated and stamped by the doctor's office.
  3. Blood Lead Testing Certificate to be completed and signed by a parent and/or doctor. 
  • Immunization Chart (Doctor offices may complete the information directly on the form or provide a printout on office letterhead. Regardless of the format used, all documents require the doctor's signature, date and stamp.)
  • Topical Basic Care Product Application Authorization Form Authorization for the ECC Educators to apply topical basic care products such as a diaper rash product, sunscreen, or insect repellent, and non medicated hand lotion/cream or lip balm authorized and supplied by the child’s parent. 
  • Medication Administration Authorization Form A separate form must be completed for each medicine to be kept on site. A doctor's signature is required on each form for all prescription and over the counter medication used to treat an issue.  All prescription and nonprescription medicines must be in the original container with any labels intact and be within the current use date. Prescription medicines must also have the pharmacy label intact. 
  • Allergy Action Form and Asthma Alert Form To be completed and signed by both a parent and doctor, as applicable to each form. These forms are required for any child with a listed allergy or asthma condition noted on their Health Inventory.
